"The fourme of byddyng of the common prayers," as used in a parish church in the time of Edw. VI.

Summary
The date of the prayer is 1547, since it comprehends a petition for the union of England and Scotland by the marriage of the king "with the yong quene of Scotlande."
"Quene Katerine dowagier" and the king's sisters Mary and Elizabeth are mentioned, and "Master Mayre of thys cytie with all hys brethern." Prayers are also desired for the "soules of my father and mother," and of "Master John Colmane, Elizabeth his wyffe, Master Thomas Bele, Alys his wyffe, Richard Knepe, quondam rector hujus ecclesie, Richard Warren, John Dyxon, and for all Chrysten sowles."
The prayers for these departed persons have been subsequently erased, and the words "pray for all them that be departed" altered into "prayse God for," etc.
